The holes in the thimble's indentations let moisture escape, permitting the evaporation of sweat. While you may not be visibly sweating, a solid thimble can hold in enough moisture to make your thumb look as pruney as it would after an hour in a bath tub. Leather is sometimes used in place of metal because it covers the thumb completely while letting the skin breath.

You may have also seen thimble with a large hole on the top. These "open" thimbles are designed to fit over thumbs with long nails.
