Medicaid is a state-run program. This will be handled by the office that handles food stamps and other assistance. in most states, this will be the Office of Health Services or the Office of Social Services.
You will need to fill out an application form and perform an interview with this office. You will be expected to show proof of income, medical bill receipts, and health insurance information.
To qualify, you must be broke. Your income must be very low, and while you can probably own a house, you may not qualify if your car is over a set value or is less than five years old. If you currently receive Supplemental Security Income (aka disability,) you will probably qualify.
In some cases, you may be given Medicaid with a "spend-down." Medicaid won't be available to you until you spend a certain amount on medical expenses. Unlike a deductible, this takes into account both your income and current liquid assets.
