Points to the first answerer for vocabulary technicalities...yes, if someone survived, then it certainly wasn't lethal :):)
There are a few question and answer sites out there claiming that someone survived lethal injection twice and also survived the electric chair....but, I searched the web and even a few library databases and I do not find a single news article supporting this claim.
So, no, no one has ever survived an execution by lethal injection.
Lethal injection usually involves three or more different injections. Usually the first injection is meant to render the person unconscious or nearly unconscious, the second injection is to stop muscle movement, and finally the third injection is potassium chloride to end the person's life. Some states do it differently, but that's a pretty fair summary of the basic process.
If anyone survived all that, I'm pretty sure it would be headline news for a long time, so you and I would hear about it in the mainstream media and not on obscure Q&A sites like this one :)
